Component "TVS" on MiniProg3 unit burned out on second day of use. What could it be?

Using MiniProg 3 to program PSOC1. Component "TVS" on MiniProg3 unit burned out on second day of use. Looking to find possible reason this would have happened. Can the orientation (putting the programmer pins on backwards) cause this? Can it be related to a short in the usb cable we are using to hook up to the computer (using an extension type usb, not the one provided in the kit)? Any feedback would be greatly appreciated so that we don't burn the next one out.

We also met similar issue before, my experience is:

(1)  Avoid short Miniprog3 Vtarg to GND of target board or vice versa.

(2)  Avoid power the target board use external power source and Internal power source at the same time.

(3)  Don't plug Miniprog3 with target board after target board is powered up.

We have experienced the same thing! Our Tech, had the internal power on the mini-prog active, and an external source active at the same time.
