What does test_enable in BLE_PROFILE_CFG mean?
All sample application enables it as 1.

I spoke with the applications team and it appears that the test_enable struct member is not used by the stack anywhere. It's mainly for application’s use when the application needs to run different pieces of code under test (so it can be enabled/disabled over the air by the peer device). In house we've used this feature to test some of the reference apps in the SDK.
